We'll be in touch soon!The Licensed Practical Nurse (LPN) will be responsible for performing restorative nursing procedures that maximize the resident's existing abilities, emphasize independence instead of dependence, and maximize the negative effects of disability with an attitude of realistic optimism under the supervision of a restorative nurse.
ESSENTIAL DUTIES AND RESPONSIBILITIESAssist in developing restorative care plans for individual residents in coordination and conjunction with the resident assessment (MDS)Review the restorative care requirements of each resident admitted to the facility and assist the attending physician in planning for the resident's care and/or treatmentEnsure that care plans indicate restorative care when therapy is orderedParticipate in the development and implementation of care plans, including quarterly reviewsReview request for restorative care and physician' ordersInterview resident to determine type of treatment and schedule therapySelect rehabilitative and therapeutic activities and exercises based on medical and social history of residentsEvaluate type of therapy most desirable after consulting with the physician and therapy personnelParticipate in the development and implementation of resident assessments (MDS) and care plans, including quarterly reviews, as they relate to restorative careComplete assigned portions of the MDS as assignedEnsure that the MDS is completed and signed in a timely manner to ensure that MDS can be transmitted in accordance with established policyBrief resident of procedures involved in restorative care and prepare necessary equipmentAssist in transporting resident to and from therapy room/areaDemonstrate to residents and staff personnel, as necessary, the procedures involved in treatment processPerform restorative care treatments in resident's room as necessaryPrepare resident for treatment by dress/position and administer restorative care in accordance with established policies and proceduresEncourage residents to perform or participate in ADL functionsEncourage residents to perform range of motion exercisesInstruct and supervise resident during therapyRecommend modifications and changes in the resident's restorative care as indicatedAssure that all restorative care notes are informative and descriptive of the treatment provided and of the resident's response to the carePHYSICAL DEMANDSWhile performing the duties of this job, the employee is frequently required to stand and walk.
The employee is occasionally required to sit, use hands and fingers, handle, or feel reach with hands and arms; talk or hear, and taste or smell.
The employee must occasionally lift and/or move up to 50 pounds.

The noise level in the work environment is usually moderateENVIRONMENTAL CONDITIONSBecause the essential functions of the job of the Licensed Practical Nurse (LPN) often require exposure to bodily functions and infectious waste, disease, odors and other situations associated with health care, including but not limited to exposure to AIDS, HIV and Hepatitis B viruses.
Involving Risk Classification Categories 1, 2, and 3, environmental conditions will fluctuateEDUCATION AND EXPERIENCE REQUIREMENTSGraduate of an accredited college or university of Nursing or be a graduate of an approved LPN/LVN program.
Valid State Specific licensure as an LPNMinimum 1 years of experience as a restorative care in a long-term care environmentStrong management and organizational skillsThorough understanding of regulatory process, MDS and care planning processTraining in rehabilitative and restorative therapy practicesMust know documentation requirements and be driven by quality care standardsAbility to work with physicians and other healthcare professionalsProficient in the use of personal computer Recommended Skills Assessments Care Planning Coordinating Licensed Practical Nurse Long Term Care Nursing Estimated Salary: $20 to $28 per hour based on qualifications.
